Sunday Worship At Fresh Start! *** Please Be Advised That If You've Traveled To A Restricted State/Country, YOU MUST QUARANTINE FOR 14 DAYS IN ACCORDANCE WITH NYS REGULATIONS - We Want To Keep Our Members Safe & Our Doors Open, Please Adhere To This! *** LET'S WORSHIP! We are so excited to worship with you all. This ticket is for your designated Sunday. If you plan on attending in-person Worship for this scheduled event, please register as soon as possible. *Please be aware that registering for this event also gives us consent to capture and release any photos or videos that will be captured during this event* You may also register guests. Please note that all guests will be taken into consideration following membership registration, depending on seat availability. **DOORS OPEN AT 1:30 PM and CLOSE AT 1:55 PM**When registering, please be sure to register your family as well (children and spouse) if all are expected to attend. There will be a follow up call the Saturday before your scheduled Sunday to confirm attendance. We ask that you lookout for this and disclose your availability as best as possible so that we can leave room for guests and visitors. All safety regulations and procedures will be taken upon arrival including but not limited to, temperature checks, mask requirements, and consent forms. We ask that you are diligent on arriving on time as we would like to get you inside and seated as quickly and safely as possible. Release of Liability While participating in events held or sponsored by Fresh Start Christian Center (“FS”) “social distancing” must be practiced and face coverings worn at all times to reduce the risks of exposure to COVID-19. Because COVID-19 is extremely contagious and is spread mainly from person-to-person contact, FS has put in place preventative measures to reduce the spread of COVID-19. However, FS cannot guarantee that its participants, volunteers, partners, or others in attendance will not become infected with COVID-19.​ ​In light of the ongoing spread of COVID-19, individuals who fall within any of the categories below should not engage in FS events and/or other face to face fundraising activities. By attending an FS event, you certify that you do not fall into any of the following categories: 1. Individuals who currently or within the past fourteen (14) days have experienced any symptoms associated with COVID-19, which include fever, cough, and shortness of breath among others; 2. Individuals who have traveled at any point in the past fourteen (14) days either internationally or to a community in the U.S. that has experienced or is experiencing sustained community spread of COVID-19; or 3. Individuals who believe that they may have been exposed to a confirmed or suspected case of COVID-19 or have been diagnosed with COVID-19 and are not yet cleared as non-contagious by state or local public health authorities or the health care team responsible for their treatment. DUTY TO SELF-MONITOR:​ Participants and volunteers agree to self-monitor for signs and symptoms of COVID-19 (symptoms typically include fever, cough, and shortness of breath) and, contact FS at [email protected] if he/she experiences symptoms of COVID-19 within 14 days after participating or volunteering with FS.
