Moderna COVID-19 Vaccine Registration for Thursday, February 18, 2021, at the Walton County Fairgrounds The coronavirus immunizations for Florida Residents ages 65 years and older or those that are frontline medical workers with direct patient contact providing services in Florida. They will be provided at the Walton County Fairgrounds (790 N 9th St, Defuniak Springs, FL 32433) by appointment only. You must be 65 years or older the day you register to receive your first dose of the vaccine. Your age will be verified by FDOH- Walton. If you are below the age of 65 your appointment will be cancelled. Those with a underling medical condition making them extremely vulnerable to COVID-19 can only be vaccinated by a hospital provide. Scared Heart Emerald Coast has been receiving doses for this purpose. DOH-Walton cannot vaccinate anyone under this provision of the Executive Order. VACCINATIONS ARE BY APPOINTMENT ONLY. YOU MUST BRING THE BELOW INFORMATION TO THE VACCINE SITE. FAILURE TO PROVIDE THIS DOCUMENTATION WILL RESULT IN YOU NOT RECEIVING A VACCINE. PROOF OF YOUR REGISTRATION PROOF OF AGE (65 YEARS OR OLDER)PROOF OF FLORIDA RESIDENCY PROOF OF MEDICAL LICENCE AND EMPLOYMENT WITH A MEDICAL PROVIDER (Only for those under the frontline medical worker provision)PLEASE ARRIVE NO MORE THAN 10 MINUTES BEFORE YOUR APPOINTMENT WINDOW. This distribution will be for portion of the vaccines Walton County received this week. The remaining doses will be spread out at two other distributions this week. There is no waitlist for this or other distributions. Please continue to monitor the FDOH – Walton Website, Walton County BCC Website, and the Walton County Emergency Management Facebook page for future distributions. Further immunizations will be scheduled after these initial dates as vaccine supply allows. We will be administering the Moderna COVID-19 vaccine. This means if you previously received Pfizer vaccine for your first dose you CANNOT receive the Moderna vaccine for your second dose. This also means your second dose MUST be the Moderna vaccine. As of Thursday, January 21, 2021, the Florida Surgeon General issued a Public Health Advisory which prioritizes Florida residents for receiving a 1st Dose of COVID-19 Vaccine. Those who wish to receive their first vaccine dose must provide proof of residency meeting either criteria 1, 2, or 3 below. 1. Valid Florida driver license issued by Florida State Department of Highway Safety and Motor Vehicles (FLHSMV). - OR – 2. Valid Florida identification card issued by Florida State Department of Highway Safety and Motor Vehicles (FLHSMV). - OR – 3. If an individual cannot present #1 or #2, the individual can prove satisfactory Florida residency by presenting a government issued ID AND a copy of any TWO of the following items if they reflect a Florida residential address (Note: A P.O. box or commercial/business address is not sufficient and the two items cannot be from the same category): Category A: a deed, mortgage, monthly mortgage statement, mortgage payment booklet, OR residential rental or lease agreementCategory B: a utility hookup or utility work order dated within 60 days before the date of vaccinationCategory C: a utility bill dated within two months of the date of vaccinationCategory D: mail from a financial institution, including checking, savings, or investment account statements, dated within two months of the date of vaccinationCategory E: mail from a federal, state, county, or municipal government agency, dated within two months of the date of vaccinationCategory F: proof of a Florida residential address for the individual’s parent, stepparent, legal guardian, or other person with whom the seasonal resident resides in Florida, PLUS a written statement from the person with whom the seasonal resident resides stating that the seasonal resident does reside with him or her.
