No cost Moderna vaccine available - BY APPOINTMENT ONLY Dignity Health hospitals, in partnership with Dignity Health Sports Park, are offering COVID-19 vaccinations for at-risk populations who may not have access to healthcare. If you are 65 years of age or older, it is important that you receive your vaccine as soon as possible. Please review the following details and make your appointment today. Please note: • You may only sign-up one person per time slot and must have an appointment. • Vaccinations are offered as a drive-thru clinic. • Do not come for a vaccination if you are not feeling well. • Allow approximately one hour for your appointment. Additional wait time may occur. • Bring proof of age (drivers’ license, state ID, or passport). • Bring a printed copy of your appointment confirmation. • Individuals do not need insurance to be eligible for the vaccine.
