POSITION SUMMARY:  Under general supervision, the Case Manager manages rehabilitation, behavior support, or employment services provided by PRIDE, which includes providing evaluation of the client, personal development, job skills, work/service plans, and emergency crisis management.  Employees in this job class serve as a liaison with governmental agencies, employers, staff, residential service providers and family members to provide updates and required reports on the individual’s current condition and progress.  This job requires knowledge of case management, resources available to individuals with disabilities throughout the community, and the ability to determine and coordinate the necessary services and development plans for each client.  TYPICAL DUTIES: 1. *Provide counseling and support to individuals with disabilities on personal and job related issues.  Enters, updates, and maintains a database of case file information and case files in accordance with state and federal requirements such as Ability One, Source America, CARF and related legal requirements. 2. *Completes monthly, quarterly, semi-annual, and annual case management reports on clients and keeps appropriate family, residential service providers and governmental agencies appraised of the individual’s progress.  3. *Develops work/services plans that establish goals, implements plans, and evaluates progress.  Create and maintain case records that comply with PRIDE and governmental standards. 4. *Maintains relationships with external entities necessary to effectively communicate progress on individuals with disabilities. 5. Promotes and participates in networking opportunities to discuss new programs and services, or to develop pilot projects. 6. *Develops, delivers and evaluates a variety of training classes for individuals with disabilities.  These classes include anger management, assertiveness training, money management, and related behavior and skill-based classes, job preparation and training curriculum in a classroom setting. 7. *Evaluates and manages emergency or crisis situations to ensure safety.  Completes necessary incident reporting. 8. Performs other duties and special projects as assigned. * Denotes Essential Job Function MINIMUM QUALIFICATIONS: • Three or more years of experience in case management, social services, or a related field; • CPR and First Aid Certifications a plus; • Community relationships and knowledge of available funding resources; • Ability to communicate effectively both orally and in writing; to make presentations and respond to inquiries by management, team, vendors and/or customers; • Human relation skills to build effective relationships with team and internal customers; • Intermediate to advanced computer literacy including knowledge of word processing, spreadsheet, database and presentation software; • Knowledge of business English including vocabulary, spelling, and correct grammatical usage and punctuation; • Mathematical skills to include the ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations; • Ability to establish priorities and solve a wide range of business problems; • Demonstrated customer service, common sense, problem solving and analytical skills; • Flexibility to respond to changing work priorities and handle numerous projects at the same time. EDUCATION REQUIREMENTS: Bachelors in Human Resources or related field A comparable combination of formal education and work experience will be considered. CERTIFICATES OR LICENSES REQUIRED: The following licenses or certificates may be required depending on local, state and/or contract requirements: CPR/First Aid Valid Driver's License PHYSICAL REQUIREMENTS: Employees must have the ability to perform the following physical demands for extended periods of time with or without assistance: • Maneuvering to and around equipment • Bending, kneeling, stooping, and reaching • Transporting to various locations within customer site • Viewing computer screens • Utilizing keyboard WORK ENVIRONMENT:  Work is performed in a variety of locations including in a business office, customer worksite, and community environments.  Driving to and from various locations is required including transporting clients.
