Description

DANBURY, CT — The city's Department of Health and Human Services is now vaccinating area residents aged 75 and over at the Danbury Health Department Clinic. In an effort to support those residents who have had technological challenges with the Vaccine Administration Management System, including those who do not have access to the internet/email, the Health Department is calling residents directly to schedule appointments at the Danbury Health Department Clinic. Caregivers and family members of eligible residents who cannot access email or the VAMS system are urged to assist them with filling out this online form. Completion of the form will schedule them to receive a call from the Danbury Health Department to plan their vaccine appointment. Spanish and Portuguese translations are included on the online form. The Clinic, which is open by appointment only, is exclusively administering the Moderna vaccine. Eligible residents may also schedule over the phone by calling the CT COVID Vaccine Appointment Assist Line at 877-918-2224. The number is staffed seven days a week from 8:00 a.m. to 8:00 p.m. Residents who call the CT COVID Vaccine Line may ask to schedule their appointment at the Danbury Health Department Clinic. The Health Department is encouraging those eligible with internet access and email addresses to book their appointments through VAMS.
