Schedule your Moderna COVID-19 vaccine if you qualify in one of the below categories. Eligibility- Vaccinations are available in stages based on priority groups from federal and state guidance and recommendations from the County of San Diego. Effective 01/23/21, individuals who live or work in San Diego County and are in the following groups are eligible for COVID-19 vaccinations in San Diego County: Healthcare workers, First Responders, Long Term Facility Care, and all others in Phase 1A, Tiers 1-3Persons aged 65 years old and above in Phase 1B, Tier 1 Proof of Vaccination Eligibility: At your appointment, you MUST present photo ID/documentation to show you meet the eligibility requirements above to receive a COVID-19 vaccination. HEALTHCARE WORKERS & OTHERS IN PHASE 1A: Employee photo ID badge; or Professional license AND a photo ID; or Signed letter from your employer on facility letterhead AND a photo ID; or Payment stub or timesheet from your healthcare employer or in-home supportive services AND a photo ID. PERSONS AGED 65 YEARS OLD & ABOVE: Driver's license; or Government-issued photo ID with your date of birth, such as a passport How to Prepare for your Appointment: Please bring your healthcare insurance card, if applicable, and REVIEW OUR WEBSITE for a Moderna Vaccine fact sheet, PLUS print and complete the SD Vaccination Immunization Registration form and our Screening/Authorization form .
