The WPB Chamber of Commerce in conjunction with Northshore Clinical Labs will be hosting FREE Covid-19 Testing @ Debonair Social Club! EVERY WEDNESDAY, 3PM-7PM As we approach the return to the City of Chicago's Tier 2 Covid Mitigation and the reopening of restaurants, bars, and other places of social gathering, it's imperative that we all do our due diligence to keep those doors open. We can all do our part by getting tested, and getting back to work again! This comes at ZERO cost to you! Just bring your Insurance Card or VALID ID! PLEASE FILL OUT THE ATTACHED FORM AND DON'T FORGET TO INCLUDE YOUR EMAIL ADDRESS! For uninsured individuals, please write your state issued ID number or SSN in the field titled "BAR CODE LAB USE ONLY." The phone number and email address provided will be used to report results.
