I need a form made in Open Office

I need a form designed to keep customer information, I need it to be a database so that I can look customers up by name, city or phone number, I also need to be able to edit information as necessary. I don’t have Microsoft Office I don’t think so I use Open Office. I have attached a sample of what my idea looks like. Reasonable price!! I need something more simple than the system they’re using.
