COVID-19 Vaccination Drive Thru for Priority Groups (65+ and health care workers with direct patient contact) Vaccination is for anyone ages 65 and older and health care workers with direct patient contact. This includes anyone born in January 1956 or earlier. Anyone under age 65 will not be vaccinated without proof of health care worker status**. This could be a badge, pay stub with ID or some other form of identification. Appointments are required for this vaccination clinic and will not be scheduled on site. Do not arrive at the vaccination site without an appointment. Appointments are scheduled to minimize wait time and control traffic. Please arrive at your appointment time. Each person receiving the vaccine must sign up for an individual appointment ticket. Bring the ticket with you to receive your vaccination. Each person must bring a form of identification (Driver's license, passport, etc.) Individuals receiving the vaccine must complete a vaccine screening and consent form to confirm eligibility. These are available on the DOH-Okaloosa website or the Okaloosa County website. For those that cannot access the forms in advance, a paper copy will be provided at your appointment time. FIRST DOSE JANUARY 15, 2021, SECOND DOSE FEBRUARY 5, 2021 Second dose occurs at same location and ticket time on February 5, 2021 This is a drive thru only, please follow prompts on-site and do not exit your vehicle unless asked to do so by staff. You may purchase up to 4 tickets per transaction. A registration/order form is required for each individual person. To purchase multiple tickets the menu must show more than one available ticket. Due to high demand, tickets may be "sold" before your sale is complete. Completed sales display an order number and you will receive a confirmation email. Tickets are also available by phone until sold out. To schedule by phone please call 850-344-0566. Please note callers may experience delays, dropped calls and busy signals due to high call volume. Tickets are non-transferable. **Health care workers in hospitals, assisted living and long term care facilities are not eligible. Health care workers in these settings will be vaccinated by hospital, CVS, Walgreen's or another contractor. Healthcare workers in other settings must be patient facing to qualify.
