Business must be independently owned and operated, must have an actual place of business in the Metropolitan Statistical Area (Miami-Dade, Broward, Palm Beach), at least 1 full year in operation, Local business tax receipt, and all required licenses, including required professional licenses, the owner is a person born in the United States, naturalized in the United States, or a permanent U.S. resident. • What required documents must I have on hand for the application? Affidavit of Certification Copy of Florida Driver’s Licenses of all owners Proof of Business structure (Articles of Incorporation) Current Lease Agreement, Purchase Agreement, or copy of Warranty Deed to show ownership of property. Copy of Birth Certificate or U.S. Passport Business Tax Returns (3 most recent years) Copy of Bank signature card for the business account or letter from the bank stating all persons authorized to sign on the account. Copy of business occupational license/Business Tax Receipt Statement of all owners’ duties within the business Copy of all owners’ resumes Copy of U.S. Passport (For foreign-born Naturalized U.S. Citizens) or Foreign Passport if not U.S. Citizen • How long does it take to process my application and become Certified?
