Parking is available at the Cone Deck - 8921 University Rd, Charlotte, NC 28262 Date: Saturday, May 6, 2023 Time: 9am - 1pm Location: 9201 University City Boulevard Charlotte, NC 28223 Parking is available at the Cone Deck - 8921 University Rd, Charlotte, NC 28262 GENERAL RELEASE FORM FOR EVENTBRITE OR OTHER ELECTRONIC REGISTRATION By participating in the 2023 Mental Health Symposium event, you agree to adhere to all instructions, guidelines, and safety rules. In addition, you acknowledge that participation in the program exposes you to a possibility of personal injury. You, being fully aware that participation in the program exposes you to a possible risk of personal injury, hereby release Alpha Kappa Alpha Sorority, Incorporated® Rho Psi Omega Chapter from any and all liability from property damage, personal injuries, or other claims arising from or in connection with your participation in the program including claims that are known and unknown, foreseen and unforeseen, future or contingent. You further release Alpha Kappa Alpha Sorority, Incorporated® Rho Psi Omega Chapter from any and all liability from any injury, damage or loss sustained by participants, guests or vehicles before, during or after the Founders’ Day 2023 event as a result of your participation in the event or associated events (this includes food or beverage consumption prepared and distributed). By participating in 2023 Mental Health Symposium event or as the parent/legal guardian of the participant in the 2023 Mental Health Symposium event, you authorize Alpha Kappa Alpha Sorority, Incorporated® Rho Psi Omega Chapter and those acting for and with Alpha Kappa Alpha Sorority, Incorporated® Rho Psi Omega Chapter to use interview, photograph, video and/or audio record you, without compensation in any medium, including print, electronic (web-based), social media material for educational, promotional and marketing purposes. This authorization shall extend to all languages, media, formats and markets now known or later discovered. This authorization shall continue indefinitely unless you revoke this authorization in writing. You waive your right to inspect or approve any finish product in which your likeness is used. You will make no monetary or other claim in connection with the authorized use of your name or photos, video, interviews, and quotes. You acknowledge that you have read and fully understand the General Release Form. You release Alpha Kappa Alpha Sorority, Incorporated® Rho Psi Omega Chapter and their partner organizations and employee of all liability. This agreement is a release of all liability and a contract enforceable against you (and your heirs, legal representatives, guardian, executors, successors, and assigns) in a court of law. Alpha Kappa Alpha Sorority, Incorporated® Rho Psi Omega Chapter reserves the right to revise these regulations and/or terminate this agreement at its sole discretion. Alpha Kappa Alpha Sorority, Incorporated® Rho Psi Omega Chapter reserves the right to remove any displays or deny entry to any group(s) that do not meet the standards set forth by Alpha Kappa Alpha Sorority Incorporated® Rho Psi Omega Chapter and their partner organizations employee by in the guidelines. You understand that there are no refunds or rain checks due to inclement weather or any other circumstances.
