Salud is grateful to host Healing Our Roots, our first official annual fundraiser social to secure venues and spaces to lead conversations to normalize mental health in Chicago. Join us in Pilsen on Wednesday, 5/17 at 6:30 PM. All are welcome to attend with a $25 fundraiser. Sponsorships are welcomed. RSVP only and required to respect capacity and space. To raise sufficient funds, a $25 suggested donation per person is noted. Only donors with such donations will be added to the list as a thank you. By RSVP'ing, registering, and attending the event, Salud and Salud presents Healing Our Roots, I understand, agree, and consent to the following: In consideration of the risk of injury that exists while participating in Salud presents Healing Our Roots (hereinafter the "Activity"); and I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives (hereinafter collectively, "Releasor", "I" or "me", which terms shall also include Releasor is under 18 years of age), knowingly and voluntarily enter into this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind arising out of my participation in the Activity; and I hereby release and forever discharge Salud and Salud presents: Healing Our Roots, located at, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors, and assigns (collectively "Releasors"), from any physical or psychological injury that I may suffer as a direct result of my participation and consumption in the aforementioned Activity.
