Joining us for worship service in-person, means you agree to the following: “Warning: Under Georgia law, there is no liability for an injury or death of an individual entering these premises if such injury or death results from the inherent risks of contracting COVID-19. You are assuming this risk by entering these premises.” If you are sick, please remain at home.Families must be seated together.Masks are required in the sanctuary.Personal PPE is also encouraged (hand sanitizer, masks, gloves)Please plan to bring and use your personal Bible/Bible App/Pens, etc...I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Fairfield Baptist Church - Auburn, GA has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I further acknowledge that Fairfield Baptist Church - Auburn, GA can not guarantee that I nor my children will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, church staff, and other church members or visitors and their families. I voluntarily choose to worship in-person at Fairfield Baptist Church - Auburn, GA and acknowledge that I am increasing my risk and that of my children to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending any services. I hereby release and agree to hold Fairfield Baptist Church - Auburn, GA harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the church, or that may otherwise arise in any way in connection with any interactions from my attendance at Fairfield Baptist Church - Auburn, GA. I understand that this release discharges Fairfield Baptist Church - Auburn, GA from any liability or claim that I, my heirs, or any personal representatives may have against the church with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any interactions at Fairfield Baptist Church - Auburn, GA. This liability waiver and release extends to the church together with all members, deacons, the pastor, and employees.
