As forum host, the Hampton Roads Community Foundation invites you to join the leadership of eight community organizations who will present an emerging regional shared agenda they are developing with a grant from Sentara Healthcare to improve health equity and literacy in Hampton Roads. The program also features a keynote from Live Well San Diego, one of the Nation’s foremost regional health collective impact programs.
