Research shows that Interdisciplinary case
management is necessary when clients
present problems that do not fall under the
authority of just one discipline. This is
exactly what we see in homeless services:
clients with needs that require expertise in
housing navigation, emergency services,
mental and physical health, employment &
disability services, eviction prevention,
criminal record sealing, the list goes on.
This is the “our client” model, rather than
“my client” and in this way goals and plans
are more condensed, specific and supported
across the team.
