Dancing, desserts, photo booth, and fun await!! Prom will be hosted at the 502 East Event Center in Carmel, IN and the event will run from 7pm-9pm. This is the permission/waiver form & Drivers permission form. Please complete this DV and this form will be good for the remaining school year of 22/23. There will be no need to submit another form for other events.
