Closing Remarks -------------------------------------------------------------------------A Tale of Two Cities: A House Call Social Worker’s PerspectiveSpeaker: Marina Nellius, LCSW-C, Community SW, Medstar House Call ProgramABSTRACT: The Social Determinants of Health (SDoH) play an important role in physiological and emotional health outcomes, particularly amongst the older population. It is essential to properly identify and understand factors that contribute to the challenges this population is experiencing. Social workers or Case Managers can influence the impact of SDoH on our aging population through evidence-based interventions, programs, and community resources to address such thus improving their quality of life or health outcomes. This session prepares a plan of action to apply strategies and overcome common obstacles to wellness confronting two different cities. Ethical principles will be explored, screening tools provided, and case studies will be presented enhancing cultural awareness for Healthcare Workers.LEARNING OBJECTIVES:1. Describe the connection of social determinants to physiological and emotional health outcomes for older adults.2. Provoke discussion on the influence of social workers and case managers on social determinants of health.3. List the most common ethical issues and dilemmas when serving two different cities. Then explore evidence-based interventions and community initiatives to address social determinants of health factors and conditions-------------------------------------------------------------------------Collaborating with Acute Care Facilities to Mitigate Discharge BarriersSpeakers: Jheni Libongco-Barbour, CEO and Founder Continuum Healthcare NetworkMitch Markowitz, Vice President, Business Development, Family & Nursing CareRachel Cicconi, Director of Marketing and Community Relations, Aarondale Retirement CommunityABSTRACT:Three Community Partners speak about their programs that offer assistance for challenging patient discharges: one Skilled Nursing Facility Organization, one Assisted Living, and one Private Duty Agency. Each comes from a different perspective in their care for people who are unable to access an insurance benefit or have a perceived barrier to the care need that is recommended by the hospital care teams. Vita Healthcare will talk about its Asian American Pacific Islander (AAPI) program. Sunrise Senior Living will speak about its Road to Home program. Family and Nursing Care will speak about their successes in having homecare CNAs in the home. Programs like these are available in all these types of organizations. Post-acute community partners are assisting to discharge patients safely and timely from the hospital, ensuring that patients are in the correct level of care.LEARNING OBJECTIVES:1. Relate learnings to their local post-acute community partners and to their leadership for potential initiatives in their area.2. Evaluate shared data proving benefits of the individual programs for patient populations.3. Share and describe learnings with colleagues to help even more patients in their facilities
