This CEU will provide a general overview of transitional planning used by Healthcare Social Workers to transition patient's across the continuum of care. The presenter for this CEU is Christopher Jones, MSW. The event is approved for 2 social work CEU hours by the West Virginia Board of Social Work. This is an in-person event hosted at Weirton Medical Center. At the time of this publication WMC continues to follow CDC guidelines for hospitals and requires everyone entering the building to wear face coverings "masks" in all public areas. As CDC guidelines change the hospital policy may be updated. Attendees of the program will be required to follow hospital policy. Please contact us if you have questions.
