Full Time: Monday- Friday Hours: 8am-4pm GENERAL SUMMARY OF POSITION The primary focus of the case manager is to utilize a trauma-informed approach to help families be prepared as possible to move into permanent housing, and to connect families to a support network of people and programs once they are housed. A case manager will work with a caseload of up to 30 families (this may increase temporarily when there is a staffing shortage) with a focus on immediate needs to support obtaining stable housing. The Case Manager will work collaboratively with the Housing Search Worker on goals to place families. * Offering an additional $500 on top for Spanish or Haitian Creole Speaker ESSENTIAL JOB FUNCTIONS* Assess families’ strengths, needs and barriers to housing in collaboration with the Housing Search Worker Resolve or mitigate tenancy-screening barriers, such as securing IDs, birth certificates, social security cards etc. Provide comprehensive advocacy, including but not limited to: Assist in enrolling in entitlement benefits (SNAP, TAFDC, SSI) Assist families in securing connections to healthcare services (e.g., community health clinics) Assist families in securing connections to job training/job placement services Work with parents to address the educational and health needs of children. Arrange family conferences/reviews with other service providers Assist families with all aspects of admission to the program including but not limited to: Reviewing house rules and site inventory Determining immediate emergency needs Completion of all necessary program documents Monitor and track progress made on their Rapid Re-Housing Plan Properly document all incidents, correspondence, needs and accomplishments of the family Conduct “unit assessments” to ensure the orderliness and cleanliness of sites as well as compliance by families with program policy When requested, assist the Program Director with training and orientation of other program staff, volunteers, or interns Able to relate to people; written and verbal communication skills Actively schedule meetings and set appointments with families Provide written biweekly summaries of case management advocacy goals and accomplishments, barriers and strategies Maintain accurate demographic and service statistics for each family On-call rotation Perform other related tasks as assigned. COMPETENCIES AND QUALIFICATIONS Bachelor’s Degree preferred. High School Diploma/GED required. Two years experience working in housing service, independent living programs or previous case management experience. Candidates must possess strong communication and organizational skills.
