Are you interested in providing the CDSMP for members of your community with chronic diseases to help them increase their ability to manage their illnesses on their own? This training will prepare you to co-facilitate (with a peer) a 6-week workshop series to provide the Chronic Disease Self-Management program and is a great opportunity to join a supported network of other leaders offering this program Oregon. Workshops can be delivered in person or virtually. Do you have a chronic disease or are you a caregiver of someone with a chronic disease? The program is lay-led and, thus, you do not need to be a health professional to lead the workshops- in fact, Stanford recommends that individuals living with a chronic condition or a caregiver for someone with a chronic condition consider being a leader. Can you commit to offering at least one 6-week program within one year, preferably within 6 months of being trained? It is really helpful to lead a training soon after to solidify your learning and be approved to lead. By doing so, you will become certified as an official Living Well Leader. What is CDSMP? CDSMP (Chronic Disease Self-Management Program) is an evidence-based program, developed by Stanford University, and is known as “Living Well with Chronic Conditions”. Workshops are facilitated by two trained leaders; one or both living with a chronic condition. Workshops are offered once per week over a six-week period for 2 ½ hours in a community setting (such as a library, community/senior center, or church). Workshops cover techniques to deal with issues such as frustration, fatigue, pain, and isolation; appropriate exercise for maintaining and improving strength, flexibility, and endurance; appropriate use of medications; communicating effectively with family, friends, and health professionals; nutrition; and how to evaluate new treatments. It is the process in which the CDSMP is taught that makes it effective. Sessions are highly participative, and mutual support and success build participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. What about training? Join other volunteer facilitators with Northwest Senior & Disability Services, RVCOG, and La Clinica by attending this four-day training. This is a free opportunity that can add to your skill set and allow you to make a difference in your community by helping others cope with chronic conditions. Co-Facilitators work in teams of two, so consider attending with a friend. It is strongly recommended that facilitators are individuals living with chronic conditions to be peer leaders, rather than health or social service professionals. Training includes some reading and home study for practice teaching with peers in class. What is required of me if I do apply? The minimum requirements for becoming a leader: 1) you must complete registration on this site, 2) complete the full 4-day training (32-hour s total) in order to become a facilitator, and 3) agree to co-facilitate a minimum of one, six-week workshop per year for NWSDS, RVCOG or La Clinica to attain and maintain your certification. The minimum commitment for leaders is to co-facilitate one six-week session per year under a licensed organization such as Clackamas County Social Services or another licensed entity. How do I apply?
