Concrete slab overlay to level

I need concrete poured over existing slab to level both floor sections. Images of both sections have laser lines on walls so you can have an idea of how many yards of concrete are needed. Diagram also includes dimensions of each room and floor height variations. Rocks and concrete chunks were placed with rebar on deepest sections. Will possibly need some 3x7 rebar mesh to lay over flat sections. I will roll bonding adhesive on the 15 x 15 flat section which only needs 1" pour. Text your labour cost estimate and availability. If you haven't done overlays before, please don't respond. Please have your own rakes, trowels and bull float, I have long 2x's if needed. No power trowel needed, I need it leveled not wavy!
