VIRTUAL FOCUS GROUP Screening Questionnaire On Tuesday, December 13, 2022, beginning at 10:00 a.m. local time for counties in Western Kentucky and lasting until approximately 4:00 p.m., we will be holding a Virtual Focus Group via Zoom. This is a mock jury where people will give opinions regarding a case. IN ORDER TO QUALIFY FOR THE VIRTUAL FOCUS GROUP, YOU MUST LIVE IN ONE OF THE FOLLOWING COUNTIES IN KENTUCKY: WARREN, BUTLER, EDMONSON, BARREN, ALLEN, SIMPSON, LOGAN, GRAYSON, HART, MUHLENBERG OR OHIO. All potential participants MUST return this screening questionnaire and complete a Confidentiality Agreement (this will be sent to you if you are selected to be a member of the mock jury) to be considered. All forms must be returned to our office by December 9th. To participate, you MUST have access to the internet and either a computer with a video camera and microphone or a smartphone with the Zoom app. You will need to be in a distraction free room in order to participate. There will NOT be an in-person session and anyone arriving at our office will not be admitted. You will receive a Zoom video conference link at 9:30 a.m. on December 13th via email. You must log-in no later than 10:00 a.m. Anyone attempting to log in after 10:00 a.m. will not be allowed to participate. The pay is $100.00. Payment in the form of a check will be mailed to your address listed below at the end of the session. To apply, you must complete the following questions.
