This year's ASID CT Holiday Party is a MUST attend! We are taking over The Spread in South Norwalk so get ready to celebrate.'Entry Tickets' will provide entrance to the event & unlimited hors d'oeuvres, full buffet dinner, two drink tickets (additional drinks cash bar) and desserts! TICKET PRICES WILL INCREASE DECEMBER 12TH So reserve your ticket now!For any questions please reach out to [email protected] Waiver and Release In consideration for my attendance at the ASID CT event, the receipt and sufficiency of which is hereby acknowledged, I hereby agree to the following: • The Coronavirus/COVID-19 is highly contagious and dangerous and there is risk of contracting Coronavirus/COVID-19 if I attend the event; • ASID CT cannot guarantee that I will not become infected with, or spread to another attendee, the Coronavirus/COVID-19; • The risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of ASID CT and the host for the event, and their officers, directors, employees, agents, and contractors, or from myself and other attendees, during the ASID CT event. Additionally, I attest that: • I, as well as all household members, are not experiencing any Coronavirus/COVID-19 symptoms, such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell; • I, as well as all household members, have not been diagnosed with Coronavirus/COVID-19 within the past 10 days; • I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19; and • I am following all applicable state and local rules and regulations related to the Coronavirus/COVID-19. Therefore, I hereby release and agree to hold the American Society of Interior Designers, Inc., and ASID CT, the host for the event, and their officers, directors, employees, and agents, harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, judgements, expenses and compensation for damage or loss to myself and/or property that may be caused by any negligence, act, or failure to act of the ASID CT, or that may otherwise arise in any way in connection with my attendance at the event. I understand that the American Society of Interior Designers, Inc. is not hosting or providing this event. Furthermore, I understand that this waiver discharges the American Society of Interior Designers, Inc., and ASID CT from any and all liability or claim that I, my heirs, or any personal representatives may have against the American Society of Interior Designers, Inc., and ASID CT with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, my attendance at the event. ASID CT does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance. I expressly waive any such claim for compensation or liability on the part of ASID CT in the event of injury, illness, or medical expenses incurred by me. I HAVE CAREFULLY READ THIS WAIVER AND RELEASE; I FULLY UNDERSTAND THE TERMS CONTAINED IN IT; I UNDERSTAND THAT, BY SIGNING, I AM RENOUNCING SUBSTANTIAL RIGHTS; I AM SIGNING FREELY AND WITHOUT CONSTRAINT; AND I WANT MY SIGNATURE TO CONSTITUTE A FULL DISCHARGE FROM ANY LIABILITY WHATSOEVER TO THE MAXIMUM EXTENT PERMITTED BY LAW. Electronic Signature. This Waiver and Release may be executed electronically, and I consent to sign this Waiver and Release in electronic form by purchasing a ticket to this event. I acknowledge and agree that my electronic purchase will have the same force and effect as an original signature for all purposes.
