Lonestar Speech Therapy is so excited to announce that our annual Christmas Bash for children with special needs and their families will be held at the Burleson Church of Christ, 1150 NW John Jones, Burleson, TX 76028. You won't want miss this! Often parents are hesitant to take their children, who have special needs, to the mall to see Santa. We invite you to bring your children and their siblings to decorate cookies, drink hot cocoa, do Christmas crafts and see Santa and Mrs. Claus. Let us join together as one family to celebrate the precious blessings and joys in our lives. Thank you for sharing your children with us! I understand that the risk of becoming exposed to or infected by COVID-19 at the event may result from the actions, omissions, or negligence of myself and others, including but not limited to, volunteers, and other participants and their families. I voluntarily agree to assume all associated risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limit to, personal injury, disability, and death), illness, damage, loss, claim, liability or expense of any kind that I or my child(ren) may experience or incur in connection with my children)’s attendance at the event. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge and hold harmless Lonestar Speech Therapy, PLLC, any other donors, its employees, agents and representatives, volunteers of and from the claims, including all liabilities, claims, actions, damages, costs, or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on actions, omissions, or negligence of Lonestar speech Therapy, its employees, agents, other donors, volunteers, and representatives, whether a COVID-19 infection occurs, before, during or after participation in any program.
