Need 8'x10' concrete work for small side entrance.

I have cracked asphalt on the outside side entrance of a roll-up door. I would like to get an estimate to get some concrete work done to replace that area. The area is approx. 8 feet X 10 feet and the concrete work should be good enough to withstand heavy traffic of a 3500lb forklift. Please let me know one total price to get this done that will include everything (labor, materials, etc.) and how long it will take you to get it done. Location is in San Leandro cross streets Marina and Alvarado. Thank you.
