Excelsior Health Services now offers COVID-19 vaccine appointments for eligible individuals who live or work in San Francisco. In an effort to provide convenient access to vaccination, Excelsior Health Services is offering appointments for eligible individuals who live/work in San Francisco. The vaccine we are currently offering is Moderna, which is authorized for use in people 18 and older. ELIGIBILITY Based on local guidelines, you are eligible for the vaccine if you live/work in San Francisco AND are one of the following: Age 50 or olderAge 18 and older with certain health conditionsPhase 1A/1B Frontline Workers: Healthcare workers, fire fighters, police officers, corrections officers, food and agricultural workers, grocery store workers, public transit workers, and those who work in the educational sector (teachers, support staff, and daycare workers.)*Starting April 15, individuals 18 and older with no health conditions will be eligible for vaccines.REQUIRED DOCUMENTS Please be sure to bring the following with you: Valid Photo IDProof of addressProof indicating eligibility (recent paystub, work badge, etc)Insurance card, Medicare card, Medi-cal card (if applicable)Once registered, you will receive a confirmation email--please print it out or show it on your mobile device.
